Housetraining

We need help, perhaps Kitten can help us!
Little Skippy (aka Forrest b/c we have realized he is not the sharpest tack in the box), isn't getting the drift. We keep him in a crate during the day and usually at night and take him out right away and several times when we are home, but he will still pee and poop whenever the urge hits him, even if it is a small sprinkle of pee, and if we see it we take him outside during the act.

What to do?
We tried the blue pads, but he only carries them through the house and chews them up. He even goes potty in his crate, he apparently has no 'clean dog instinct' even though he will go to the very corner of the yard to poop. I found poop in his crate that was put in his water and food bowl, and you know he didn't take his little paws and carry it there. He eats poop also!

What to do? What to do? We think he is brain damaged. All his litter mates died unfortunately.
